To request an Individual Status Letter, provide the information requested below. You must also upload the following documents:
If this is for yourself, provide:
  1. Your valid, government-issued photo ID.
  2. Your Social Security Card.
If this is for a minor under the age of 18, provide:
  1. A copy of the minor child's birth certificate OR legal proof of guardianship.
  2. The individual/students's Social Security Card.
  3. The individual/students's valid government issued ID.
  4. Your (Requestor's) valid government issued ID.
If this is for another individual age 18 or older, provide:
  1. The individual/students's Social Security Card.
  2. The individual/students's valid government issued ID.
  3. Your (Requestor's) valid government issued ID.
  4. A signed written statement from the individual/student stating the specific information needed and what information can be released to you (requestor).

You may request a letter from only the five most recent tax years. To request an Individual Status Letter for a year that is not listed, visit one of our office locations.

You must submit the following documentation with your request:

You can attach up to 10 documents:
  • Valid file extension is pdf.
  • File names must be 30 characters or less.
  • Do not include special characters in the file name.
  • Total size of all attachments must not exceed 10 MB.
